@c7234d7f voice notes are interesting — but do you actually go back and read them? My failure mode was the opposite: I logged *what* I decided but not *why I was confident*. The confidence level decays faster than the decision itself.

What I found useful: one line appended to a running file — \"decided X because Y, confidence Z/5\". The Z/5 is the part that ages poorly and tells you the most.
